You need to submit:

  • The completed and witnessed Child Passport Identity and Consent Form
  • You may need to submit other documents regarding your child’s guardianship - find more detailed information in our Consent for Children section. 

To help us help you, please:

  • Make sure the information matches your child’s current passport exactly. Have their passport with you when you complete the form.
  • Make sure you enter the correct birth surname for the child’s mother.
  • Make sure the photo meets our requirements. We accept most photos from one of our approved Digital Photo Providers. Photos taken at home are less likely to meet our requirements. If the photograph is incorrect, this will delay us issuing the passport.

If you renew an Irish passport, the previous one will be cancelled within 4 weeks of when you receive your child’s new passport.

As your child’s passport was issued more than 15 years ago, you must apply as a first time applicant on their behalf.

Lost, stolen or damaged passport

Lost or stolen

  • Make an application on behalf of your child for a new passport immediately using Passport Online. This is the fastest way to report the passport as lost or stolen and ensures that no one else can use it for travel.
  • If you find a child’s passport that you have previously reported as lost or stolen, please do not allow your child to travel on this passport. They may be stopped at an international border.  You should return the passport to the Passport Service.
  • The validity (duration) of their next passport may be less than the normal 5 year passport. If they have lost 2 or more passports, the passport validity may be less than normal but you will still have to pay the full fee. 
  • It may take longer to process the application for their next passport so make sure you have their new passport before you book any travel.

Damaged

  • If your child’s passport is damaged, do not allow them to travel on it. It may cause difficulties when they are crossing international borders.
  • If your child’s passport has been damaged you will need to return the damaged passport when you are making an application for a new passport.
  • It may take longer to process the application for their next passport so make sure you have your new passport before you book any travel.

The fastest way to replace your Passport is using Passport Online. You can use the paper based, Passport Express service but this will take longer.

Overseas

If your passport is lost, stolen or damaged while you are on holidays, you need a travel document to return home. You should contact your nearest Irish Embassy or Consulate who may be able to help.

    • Guardianship documents (if required). Guardianship and consent is a complex area - find more detailed information in our Consent for Children section. 
    • Full original civil birth certificate (version that includes the name(s) of the parent(s)).
    • If the parent’s name is now different to their name on the child’s birth certificate, they will need to link their identity to the child’s birth certificate. For example, they could have changed their name through marriage or divorce. We will require the original documents such as a original civil marriage certificate or the separation agreement, judicial separation court order, or the divorce decree certified by a solicitor or by the court.

Please note: Passport Online is the fastest way to apply for your passport. If you apply using Passport Online and meet the following criteria, you are not required to submit your child’s original birth certificate, as we will retrieve this record digitally from the General Register Office:

  • Your child’s birth was registered with the General Register Office
  • You provide your child’s PPSN
  • Your child is under joint guardianship as outlined in the birth certificate or single guardianship has been granted through a court order.

Select the option that applies to you:

  • One parent was an Irish citizen at the time of my child's birth
  • One parent was a British citizen at the time of my child's birth
  • One parent was an EU citizen at the time of my child's birth
  • Neither parent was an Irish, British or EU citizen at the time of my child's birth
  • One parent had refugee status in Ireland at the time of my child's birth

One of the parents of my child was an Irish citizen at the time of my child’s birth

Remember, we cannot return your original documents while the application is being processed. Please do not submit an original Irish passport.

Any supporting documents submitted will be returned within 20 working days of the passport issuing. You should not book any travel until you have all travel documents.

  • Guardianship documents (if required). Guardianship and consent is a complex area - find more detailed information in our Consent for Children section. 
  • If applying via paper: Full original civil birth certificate (version that includes the name(s) of the parent(s)).

Please submit one of the following documents for the Irish citizen parent:

  • A photocopy of the Irish citizen parent’s current Irish passport if this was issued before the child was born. Do not send the original.
  • The Irish-born parent’s full original civil birth certificate. The version that includes the name(s) of their parent(s). We do not accept baptismal certificates or birth certificates that do not show the name(s) of the parent(s). We do not accept laminated certificates.
    • If the parent’s name is now different to their name on the child’s birth certificate, they will need to link their identity to the child’s birth certificate. For example, they could have changed their name through marriage or divorce. We will require the original documents such as an original civil marriage certificate or the separation agreement, your judicial separation court order, or your divorce decree. This must be certified by a solicitor or by the court.
  • The Irish citizen parent’s original naturalisation certificate OR original Foreign Birth Registration Certificate OR original Post Nuptial Certificate.

One of the parents of my child was a British citizen at the time of my child’s birth

We can only begin to process the application when we receive the supporting documents.

Remember, we cannot return your original documents while the application is being processed. Please do not submit an original Irish passport. 

Please consider other options such as a birth certificate from a parent born in Northern Ireland, if applicable, rather than submitting your own passport. 

Any supporting documents submitted will be returned within 20 working days of the passport issuing. You should not book any travel until you have all travel documents.

We need your child’s original documents but we will return these to you. Please submit:

  • Guardianship documents (if required). Guardianship and consent is a complex area - find more detailed information in our Consent for Children section. 
  • If applying via paper: Full original civil birth certificate (version that includes the name(s) of the parent(s)).

Please submit one of the following documents for your British citizen parent:

  • Original British passport issued before the birth of the child. We cannot accept a photocopy. The citizenship section should state British Citizen, not British Subject.
  • The parent’s full original civil birth certificate if they were born on the island of Ireland. The version that includes the name(s) of their parent(s). We do not accept baptismal certificates or birth certificates that do not show the name(s) of the parent(s). We do not accept laminated certificates.
    • If the parent’s name is now different to their name on the child’s birth certificate, they will need to link their identity to the child’s birth certificate. For example, they could have changed their name through marriage or divorce. We will require the original documents such as a original civil marriage certificate or the separation agreement, your judicial separation court order, or your divorce decree. This must be certified by a solicitor or by the court.

One of the parents of my child was an EU citizen at the time of my child’s birth

We can only begin to process the application when we receive the supporting documents.

Remember, we cannot return your original documents while the application is being processed. Please do not submit your original passport. 

Consider other options such as a national identity card from an EU Member State, if applicable, rather than submitting your own passport. 

Any supporting documents submitted will be returned within 20 working days of the passport issuing. You should not book any travel until you have all travel documents.

We need your child’s original documents but we will return these to you. Please submit:

  • Guardianship documents (if required). Guardianship and consent is a complex area - find more detailed information in our Consent for Children section. 
  • If applying via paper: Full original civil birth certificate (version that includes the name(s) of the parent(s)).

We need your (the parent’s) original documents but we will return these to you. Please submit:

  • Original National ID card or original passport of the EU citizen parent.
  • Parent’s proof of residency in Ireland for 3 of the 4 years before the child’s birth.
    • Tax records (P60 / P45 / P35L / Employment Detail Summary ONLY) for each year showing at least 35 weeks insurable employment.  If less than 35 weeks please submit additional documents for remaining months of the year.
    • Notice of Assessment / Self Assessment records for each year and supporting documentation e.g. Bank Statements / Domestic bills for each month
    • Letter from Social Welfare/Intreo confirming type of benefit payment you received and the dates to and from when the benefit was paid directly to you, this must be accompanied by a photocopy of your PPS card front and back
    • Rent Lease (must include Contract page and Signature of the Parent whose residency we are considering) with receipts / utility bills/ rent book to confirm payments for each month
    • Bank Statements for each month showing regular transactions in and out of the account
    • School/College letters confirming dates of attendance, full time courses undertaken and proof of exam results
    • Declaration Form A: This is a sworn statement by the parent, giving details of their period of residency in Ireland. Please ensure that:
      • The word Father or Mother is deleted as appropriate from page 1 of the form;
      • The EU member State (the country of origin ) should also be specified;
      • The dates of Residency should include month(s) and year(s) both From and To;
      • The form is witnessed by one of the witnesses listed on the back of the form. It cannot be witnessed by a member of An Garda Siochána.

Neither parent of my child was an Irish, British or EU citizen at the time of my child’s birth

Remember, we cannot return your original documents while the application is being processed. Any documents submitted will be returned within 20 working days of the Passport issuing. You should not book any travel until you have all travel documents.

Your child may not qualify for an Irish passport if you lived in Ireland while:

  • Having an application for refugee status determined
  • Study or training purpose
  • Periods of unlawful residence or
  • Where your child’s parent had diplomatic immunity at the time of the birth 

We need your child’s original documents but we will return these to you. Please submit:

  • Guardianship documents (if required). Guardianship and consent is a complex area - find more detailed information in our Consent for Children section. 
  • If applying via paper: Full original civil birth certificate (version that includes the name(s) of the parent(s)).

We need your (the parent’s) original documents but we will return these to you. Please submit:

  • Original national passport of the parent showing residency stamps for 3 of the previous 4 years before the child is born. You can submit an expired passport as well.
  • The Parent’s GNIB (Garda National Immigration Bureau) card / IRP (Irish Residence Permit) card.
  • A summary statement, signed by parent of parent’s ‘permission to remain’ stamps in the passport. The summary (click here to download the template) should include:
    • Number of the passport.
    • Name of passport holder.
    • Description of the stamp(s).
    • Dates of the permission.

One parent had refugee status in Ireland at the time of my child’s birth

Remember, we cannot return your original documents while the application is being processed. The passport and Irish Residence Permit of the parent will be returned within 20 working days of the passport issuing. You should not book any travel until you have all travel documents.

Your child may not qualify for an Irish passport if you lived in Ireland while:

  • Having an application for refugee status determined
  • Study or training purpose
  • Periods of unlawful residence or
  • Where your child’s parent had diplomatic immunity at the time of the birth

  • The original letter from the Department of Justice declaring the parent's refugee status before the child was born
  • The parent's original passport or travel document.
  • The parent's original Irish Residence Permit or GNIB card.

My child was born on the island of Ireland, my child was adopted and one parent was an Irish citizen at the time of the adoption

If your child was born in Ireland and adopted by you under Irish Law, we need your child’s original documents but we will return these to you. Please submit

  • Completed  Form 
    • Guardianship documents (if required). Guardianship and consent is a complex area - find more detailed information in our Consent for Children section. 
    • The adoption certificate by the adoption authority where adoption was effected - if adopted before 1 November 2010, or the Intercountry Adoption certificate issued by Adoption Authority of Ireland if adopted on or after 1 November 2010.

We need your (the Irish parent’s) original documents but we will return these to you. Please submit one of the following:

  • The Irish citizen parent’s Irish passport at the time of the adoption. If this is still valid, please send a photocopy not the original passport.
  • The Irish-born parent’s full original civil birth certificate. The version that includes the name(s) of their parent(s). We do not accept baptismal certificates or birth certificates that do not show the name(s) of the parent(s). We do not accept laminated certificates. If the parent’s name is now different to their name on the child’s adoption certificate, you will need to link their identity to the child’s adoption certificate. For example, they could have changed their name through marriage or divorce. We will require the original documents such as an original civil marriage certificate or the separation agreement, your judicial separation court order, or your divorce decree. This must be certified by a solicitor or by the court.
  • The Irish citizen parent’s original naturalisation certificate OR original Foreign Birth Registration Certificate OR original Post Nuptial Certificate. This must be dated before the date of the adoption.

We can only begin to process the application when we receive the supporting documents.

Remember, we cannot return your original documents while the application is being processed. Please do not submit an original Irish passport. Please consider other options such as an original birth certificate, rather than submitting your own passport.

Any passport submitted will be returned within 20 working days of the passport issuing however local postal delivery may take longer. You should not book any travel until you have all travel documents.

We need your child’s original documents but we will return these to you. Please submit:

  • Full original civil birth certificate (version that includes the name(s) of the parent(s)) from the country where your child was born. If it is not in English or Irish, you must include a certified translation into English or Irish.

Please submit one of the following documents for the Irish citizen parent:

  • A photocopy of the Irish citizen parent’s current Irish passport. Do not send the original.
  • The Irish-born parent’s full original civil birth certificate. The version that includes the name(s) of their parent(s). We do not accept baptismal certificates or birth certificates that do not show the name(s) of the parent(s). We do not accept laminated certificates.
    • If the parent’s name is now different to their name on the child’s birth certificate, they will need to link their identity to the child’s birth certificate. For example, they could have changed their name through marriage or divorce. You need to apply for Foreign Birth Registration for your child before you can apply for your child’s Irish passport.

Passport applications for children under the age of 18 need the consent of all guardians. In certain circumstances, a passport application for a child may need to be accompanied by a Sole Guardian Affidavit Form.

Declaration Form A

What is it

A sworn statement by the parent, giving details of their period of residency in Ireland.

When do you need it

  • The child was born after January 1st 2005 and
  • You want to show the citizenship of the child through habitual residency in Ireland and
  • The parents were born within the European Union (EU), European Economic Area (EEA) or Switzerland (Excluding Northern Ireland and Great Britain).

You must get this form witnessed.
